Introduction

RISE-Haiti SPOTLIGHT is a social medicine, HIV Provider Stigma and Discrimination Prevention Project activity that will lead to the development of materials and approaches to be rolled out by the Government
of Haiti’s (GOH) HIV/AIDS National Program (PNLS) and implemented by health providers across the
country.
RISE-Haiti SPOTLIGHT aims to support the GOH to achieve and sustain control of the HIV epidemic by
improving the quality of HIV services, particularly HIV treatment and care, in five targeted geographic
departments (Nord, Nord-Est, Ouest, Artibonite, and Sud), to aid in retaining new and continuing patients
in treatment, thereby contributing to UNAIDS 95-95-95 goals.
Specifically, this involves:
1. Enhancing HIV provider understanding, empathy and attitudes towards people living with HIV
(PLHIV);
2. Increasing provider/patient shared decision-making processes (PPSDMP);
3. Supporting the Ministry of Health (MOH) and PNLS to develop an approved national set of HIV
provider competencies, revision/development of protocols and standard operating procedures as
they relate to improving the patient/provider relationship; and
4. Supporting advocacy efforts to improve HIV providers, medical and nursing students’ curriculum
related to the provider/patient relationship.
In Haiti, RISE will work with the MOH and the PNLS, as well as other MOH/directorates and leadership at
the departmental level, local civil society and university partners, PEPFAR implementing partners, and
professional organizations and associations to improve the quality of HIV services delivered by HIV
provider teams. By changing provider attitudes and improving the quality of care, RISE-Haiti SPOTLIGHT
will help to reduce interruption in treatment (IIT) and increase continuity of treatment among PLHIV. This,
in turn, will lead to more PLHIV on antiretroviral therapy (ART) that are virally suppressed.
Tables 1 and 2 show the distribution of the PLHIV population on ART, number of sites, and number of sites
with high IIT rates (>10%) by geographic department. RISE-Haiti SPOTLIGHT on-site support will be tailored
and customized to meet the needs of sites depending on factors including volume of PLHIV served and
performance on critical indicators including IIT and viral load suppression.

RISE-Haiti SPOTLIGHT is looking for a Haiti-based civil society organization supporting HIV services to
support program activities for nine months.
Description of Services to be provided:
The selected Recipient will be the operational branch of the project in the field and will be in close contact
with RISE-Haiti SPOTLIGHT project staff, the PEPFAR health facilities covered by the project, the
Departmental Directorate HIV focal point, and other PLHIV and community (including key populations,
youth, and other priority population) associations, networks, and advisory groups.
Capacity Development of HIV Providers to Improve Client-centered Care. To standardize the transfer of
competences, the Recipient will participate in all training of trainers aiming at enhancing HIV provider
understanding, empathy and attitudes towards PLHIV and strengthening the collaboration between
patients and providers. These trainings will be led by MSH with validated PNLS materials aiming at
reducing stigma and discrimination within the healthcare setting, reinforcing the providers/clients
relationship, and ultimately assuring that patients are maintained in their HIV care regimen. The Recipient
will plan and conduct similar training for HIV provider staff at PEPFAR health facilities that will be under
their coverage area. The targeted population for those training at the health care facilities will not be
limited only to the health professionals but will also include employees (supportive staff, counselors, data
clerk, etc.) who will interact with the patients at the site.
Coaching and Supportive Supervision. In addition to the training, the Recipient will conduct regular site
visits independently and jointly with an HIV Departmental Directorate representative. During those visits
the Recipient will coach the health facilities staff who participated at the training conducted with
validated PNLS materials. This coaching will assure competences obtained through the training are
applied and that patients are receiving stigma free services. In addition to mentoring the staff on the best
practices to adopt, they will monitor and advise the supporting health facilities staff on how to maintain
the waiting room and facilities to be welcoming for the patients. A particular attention will be paid toward
the sites with retention rates lower than 90% with the goal to contribute to increasing their rate to 90%
or higher.
Client Feedback and Service Monitoring. It will be expected from the Recipient that they also interact
with PLHIV existing clubs at the health facilities to gather feedback on the services they are receiving from
the health facilities. This may take place through a variety of methods, including site feedback or survey
activities; patient advisory group meetings; focus group discussions; and deployment of mystery client
visits (an approach whereby an anonymous PLHIV who has knowledge of HIV services and quality service
provision presents at a health facility to assess the quality of the services provided [e.g., if services are
delivered without stigma and discrimination]. The data collected will allow RISE-Haiti SPOTLIGHT to
monitor improvements in the quality of care provided to the patients. On the same note we will also
expect that the Recipient participates at the regular partners meetings conducted at the departmental
level. This will enable the organization to utilize this platform to share and discuss challenges that are not
contributing to reducing stigma within the health Institutions.
Eligibility Requirements
The selected Recipient should have demonstrated experience conducting related activities in the selected
departments covered by RISE-Haiti SPOTLIGHT Project during the last three years. The project intends to
award Fixed Amount Awards as follows:
? Cluster 1: One (1) organization that will cover the Departments of North and North East
? Cluster 2: One (1) organization that will cover sites in the Departments of Ouest and Artibonite

? Cluster 3: One (1) organization that will cover sites in the South Department
Organizations may submit proposals to cover more than one cluster and multiple departments, if
interested. Proposals (both technical and cost) must be separated by cluster.
The selected Recipient must have the capacity to conduct training and site visits, coordinate and facilitate
stakeholder meetings, and collect data that are relevant for the project and to coach staff at the health
care facilities to maintain and adopt a stigma free attitude towards PLHIV. The selected Recipient must
also have the capacity to work with the Ministry of Health at the departmental level and the PLHIV
networks, including for KP, youth, and other priority populations, in their area of intervention.
We expect that the selected Recipients will also collaborate with each other to assure a cross fertilization
of the experiences that they are having in the areas under their responsibility.
